Southampton University Hospitals NHS Trust have been fined £100,000 for failing to supervise two junior doctors when a patient died following a routine knee operation.
The trust was charged under the Health and Safety Act with not properly managing two doctors when Sean Phillips, 31, of Southampton, died after routine knee surgery in 2000.
Mr Justice Cresswell, sitting at Winchester Crown Court, said the breach was an "extremely serious offence"
